Frequently Asked Questions
Answers to the questions practices ask us most. If yours is not here, ask an RCM specialist.
General
What does a revenue cycle management company do?
A revenue cycle management company handles the financial side of patient care, from checking eligibility and authorizations before a visit through coding, claim submission, payment posting, denial follow-up, patient billing and reporting. Ace can run the whole cycle for you or take on the specific stages where your team needs help.
Will we keep our current EHR or practice management system?
Yes. Our team works inside your existing EHR, practice management and clearinghouse systems, so your providers and staff keep the tools they already use.
What is included in end-to-end RCM?
The full cycle: eligibility and authorization support, coding, claim submission, payment posting, denial management, A/R follow-up, patient collections and reporting. You can also start with the stages where you need the most help and add others later.
How do you report progress?
You receive a regular KPI report covering clean-claim rate, denial rate, days in A/R, net collection rate and A/R over 90 days, and we review it with you every month.
Pricing & Onboarding
How is outsourced medical billing priced?
Pricing depends on your claim volume, specialty and the services you need. You receive a written quote after the free RCM assessment, before any work begins.
How quickly can you get started?
Onboarding begins with discovery, system access and baseline KPIs in the first 30 days, followed by workflow set-up and backlog clean-up.
How is RCM priced?
RCM pricing depends on claim volume, specialty and which stages of the cycle we manage. We share a written quote after the free assessment.
How long does onboarding take?
Most onboarding follows a 90-day plan: discovery and baseline KPIs in the first 30 days, workflow set-up and backlog clean-up by day 60, then full operations with a monthly KPI review by day 90.
How quickly can we start?
We begin by defining the roles and hours you need, then match and train staff before they start.
Billing & Coding
Which billing systems do you work in?
We work inside your existing EHR, practice management and clearinghouse systems rather than asking you to switch.
How quickly are claims submitted?
Claims are scrubbed and submitted promptly once complete charges are received, and charge lag is part of your regular reporting.
Who handles rejections?
We do. Clearinghouse and payer rejections are corrected and resubmitted by our team, and recurring causes are reported back so they can be fixed at the source.
What reports will we receive?
Regular reports on claims submitted, payments posted, rejections, denials and A/R, plus a monthly KPI review.
Which specialties do you code for?
Our coders work across specialties with CPT, ICD-10 and HCPCS codes and follow the payer-specific requirements for each one. Tell us your specialty during the free RCM assessment and we will confirm the fit.
How do you handle documentation queries?
When documentation does not support a code, the coder sends a query to the provider through the channel you prefer and holds the claim until it is answered, so nothing is guessed.
How is coding quality checked?
Through regular quality reviews of coded claims and by tracking coding-related denials.
What are the most common denial reasons?
Missing or incorrect patient information, eligibility and coverage problems, missing prior authorizations, coding errors, duplicate claims and missed filing deadlines. Most are preventable with checks before submission.
How fast are appeals filed?
Appeals are prioritized by deadline and value, so time-sensitive and high-dollar denials are worked first.
Can you work our existing denial backlog?
Yes. We categorize the backlog, appeal claims still within their filing limits first and report what can still be recovered.
Can you take on old A/R?
Yes. We review older balances by payer and filing limit, work the ones that are still recoverable and tell you plainly which ones are not.
How do you decide what to work first?
By recovery value: balance size, age, payer and how close each claim is to its filing deadline.
How often will we see progress?
Every month in the KPI review, including days in A/R, A/R over 90 days and amount recovered.
How do you communicate with patients?
With clear statements and reminders, followed by courteous calls or messages when needed. Our staff explain balances plainly and are trained to stay patient and respectful.
Do you offer payment plans?
Yes. We set up payment plans patients can realistically manage, within the guidelines you approve.
How do you handle disputes?
Promptly. We check the claim, the payer's explanation of benefits and the statement, explain the outcome to the patient and correct anything that was wrong.
How do you protect our reputation?
Every patient conversation follows the tone and policies you approve, and sensitive situations are escalated to your team rather than pushed for payment.
Credentialing
How long does credentialing take?
It varies by payer and state, commonly from several weeks to a few months. Complete applications submitted early are the biggest factor we control.
What documents are needed?
Typically state licenses, DEA and board certifications, malpractice insurance, work history, education and training records and NPI details. We send a checklist at the start so nothing is missing.
Do you manage CAQH profiles?
Yes. We keep CAQH ProView profiles complete and re-attested on schedule so payers can rely on them.
Do you handle re-credentialing?
Yes. We track expiration and re-credentialing dates and start renewals ahead of time so providers stay in network.
Staffing
What hours do staff work?
Staff work the hours you need, including your local business hours.
How are staff screened and trained?
Staff go through screening and role-specific training in billing, coding and payer rules, plus data-security training before they access your systems.
Can we scale up for a short project?
Yes. Virtual staffing can scale up for a short project, such as an A/R clean-up or a system migration, and back down afterwards.
Security & Data
What systems do you integrate with?
Common EHR, practice management and clearinghouse systems, connected so data moves where it is needed.
How do you protect data?
With encryption, role-based access controls, staff training and regular audits.
Do you set up telehealth platforms?
Yes. We set up video conferencing and remote patient monitoring, integrated with your existing systems.
Do you offer ongoing support?
Yes, ongoing support and training for the systems we build or manage.
